The registration fee includes reading materials, continental breakfasts, lunch,
refreshment breaks, and reception. Take advantage of EARLY BIRD REGISTRATION
to receive these rates for this seminar. Rates will increase by $25.00 for registrations
received by mail or fax after 4/14/99. Please allow sufficient time for mailing. To
ensure inclusion on the attendee roster, the registrants card and fee must be
received in the Societys office by Thursday, April 22, 1999.

A confirmation of your registration will be sent by mail, however registrations received less than one week prior to the seminar date will not receive written confirmation.
CANCELLATION POLICY
All cancellations should be made by phoning the Continuing Education Department in order
to obtain a refund of the registration fee. The Society of Actuaries will refund the
registration fee, minus a processing fee of $50. Please confirm your cancellation in
writing. The Society reserves the right to cancel any seminar if conditions warrant. In
the event of such cancellations, registration fees will be refunded in full. We are not
responsible for any discounted airfares or hotel penalties that an attendee may incur due
to cancellation.
